Background: The number of diabetic patients in Korea is rapidly increasing every year, and as of 2020, there are about 6.05 million diabetic patients over the age of 30, and one in six is said to be suffering from diabetes. However, considering the number of pre-diabetic and hidden patients, it is estimated that there will be a much larger number of diabetic patients than this.
In addition, diabetes is a major cause of various complications and requires steady management to prevent it, and management programs for this exist. This study aims to find out whether these services are safe and effective for diabetes management by enabling medical staff to continuously manage the health of diabetic patients through remote monitoring services.
Objective: The purpose of this study is to find out whether a remote monitoring solution for diabetic patients is effective in managing diabetes.
Methods: Remote monitoring service for blood pressure, blood sugar, diet, weight, medication, waist/thigh circumference, and activity (step count) was provided for 3 months through the IoMT device and mobile system for 400 diabetic patients (Ex=200 / Con.=200) being treated at primary medical institutions in Gangwon province. Patient data collected in real time was monitored by medical staff and used for treatment, and customized health information was provided. The satisfaction of remote monitoring service before and after the service was evaluated from the subjects, and changes in the acquired data were compared and analyzed.
Results: From a clinical point of view, the remote monitoring service showed no significant changes in high-density cholesterol, triglyceride, cholesterol, and waist circumference, but there were significant differences in thigh circumference, low-density cholesterol, BMI, weight, and systolic/diastolic blood pressure. There was a significant difference in the degree of hemoglobin reduction. This shows that the remote monitoring service using the IoMT device and mobile system is effective when applied to patients returning to primary medical institutions. In the satisfaction evaluation of remote monitoring service, the satisfaction level was on a 5-point scale.
The score was as high as 4.3 points, which means that the remote monitoring service has a high level of service satisfaction in diabetic patients and can help diabetes management by drawing sufficient compliance. In terms of medical safety, during the empirical evaluation period, there were no cases of hospitalization due to diabetes complications or other adverse reactions, and no adverse reactions were reported.
Conclusion: There were some positive changes where the health of diabetic patients was improved through the remote monitoring service, and through this, the effectiveness of the service could be confirmed.
